When engaging a home nursing service, the starting point is typically a care assessment — an evaluation of the elder's mobility, medical needs, and daily living requirements. This helps determine the appropriate level of clinical support, whether that is wound care, medication management, post-hospitalisation rehabilitation, or general nursing monitoring. Families should be clear on the number of hours required per day or week, and whether a consistent assigned nurse or a rotational team is preferred. It is also worth discussing how the service coordinates with the elder's existing doctors or specialists, and what the escalation protocol is if the patient's condition changes between visits.
To get started, reach out to Uplift Rehab via WhatsApp, phone, or the enquiry form with a brief summary of the care recipient's condition and the hours of nursing support needed. This will allow the team to advise on the appropriate care plan and confirm current availability.
Insist on a trial period and put the care plan, hours, and rate in writing.
For anyone needing home physiotherapy visits, I would say look no further than Yan Fen and her team at Uplift. It has truly been an uplifting experience for my aunt, who has regained strength, confidence and spirit slowly and steadily. Over time, her pains and aches have decreased, and her mobility has improved under the encouraging and warm guidance of Yan Fen. The family is truly thankful!
Our experience with our physiotherapist Yan Fen was fantastic . She is incredibly easy to talk to and genuinely approachable, making it easy for us to confide in her about our concerns. Her knowledge and professionalism really shows through each session we have with her. Highly recommend!!
Uplift Rehab has been exceptional in my recovery journey! Yan Fen’s warm and affable demeanour made her very easy to work with in navigating and overcoming my mobility challenges. She’s also very astute in recognising my limitations which has ensured the pace of progress has been optimal and steady.
I highly recommend Uplift Rehab for home PT services. The process—from enquiry to appointment—was smooth and efficient. Our physiotherapist, Huisin, was professional, patient, and clear in teaching my grandma. She tailored each exercise to her ability and made her feel comfortable throughout. Excellent service!!
My mum had a really bad fall in Apr 25. Couldnt operate due to bad skin condition even after 12 days of hospitalisation. Started physio with Ms Rachel Yee who supported my mum with great patience and expertise. Rachel's positive and caring attitude coupled with her attentiveness to gradually work with my mum on different strengthening exercises and massage had contributed significantly to my mum's smooth recovery. Kudos to Rachel 💪💪💪Strongly recommend her if you need!

Uplift Rehab prices home nursing on an hourly basis, which is standard for visit-based care in Singapore. Market rates for home nursing visits typically range from S$25–60 per hour for enrolled nurses and S$60–120 per hour for registered nurses, depending on the clinical complexity of care required. Specific rates were not published at the time of listing, so contact Uplift Rehab directly for a quote tailored to your care needs. Beyond the hourly rate, confirm whether there is a minimum visit duration, whether there are surcharges for public holidays or after-hours visits, and whether consumables such as wound dressings or catheters are billed separately. Some providers also charge an initial assessment fee. See the full eldercare cost guide for Singapore →
Uplift Rehab operates as a home nursing service, which is visit-based rather than live-in. This typically suits families who need scheduled clinical care — such as wound management, injections, catheter care, or post-discharge monitoring — rather than round-the-clock supervision. Visit frequency can often be arranged from once a week to daily, depending on the care plan. If the elder requires companionship or personal care support between nursing visits, families sometimes pair a home nursing provider with a separate caregiver arrangement. Confirm the minimum visit length, whether a consistent nurse is assigned, and the replacement policy if the regular nurse is unavailable.
For home nursing providers in Singapore, the clinical staff should be registered or enrolled nurses recognised by the Singapore Nursing Board. Registered nurses (RNs) hold a degree-level qualification; enrolled nurses (ENs) hold a diploma. The nursing registration of individual practitioners is verifiable on the Singapore Nursing Board public register. At the time of listing, no regulatory licences were listed for Uplift Rehab on KakiList. Families should ask directly whether the provider is registered with the Agency for Integrated Care (AIC) as a home care provider, confirm the professional indemnity or public liability insurance held, and request the name and registration number of the nurse who will be visiting. These details are reasonable to request in writing before care commences.
